コース講習の前に書いていただく書類
下の記事では漠然と登録用紙と書いていますが、実際にはそれぞれ書式の名前があります。この用紙はコース参加者にすべて記入していただきます。保護者の署名、日付欄のみ保護者がかくようになっています。
